It is our providers’ preference all patient must have a referral for an appointment. Some insurance plans require authorization in order to be seen by a specialist. Please call the Member Services number, located on the back of your insurance card. In addition, if your Primary Care Physician is referring you, your PCP will need to fax us a referral including test results, diagnosis information, and other information pertinent to your care. Once received, we will schedule the appointment and will follow up with your PCP with details.
We have 3 locations to serve you: Charleston, Mount Pleasant and Moncks Corner. Please contact our office to discuss your scheduling needs and physician preference that’s most convenient to you, as not all of our physicians travel to all of our locations.
Please bring your insurance card, picture ID, and a list of your current medications including dosages and the regiment in which taken.
If you are a new patient, please go under Forms on our website and print all forms. You can save time if you fill them out in advance of your visit. If you are unable to print them out, we will have them here at the office for you.
Our Physicians have privileges at many of the area’s hospitals including Roper Hospital downtown, Vibra Hospital, and East Cooper Medical Center.
Please call our billing department at (843) 577-6791 ext 608 to make prior arrangements. We have payment plan options, will discuss terms of payment and do our best to help you. Please note that copayments and visit payment are due at check in for your visit unless prior arrangements have been made.
